Renewals for businesses can be completed electronically. Please find the "Renewals" tab at the top of this page for more information. If you would like to submit a paper copy instead, please use the paper renewal form.  This form will only be accepted during open renewal periods.

The Board does not accept electronic signatures or scanned copies of applications. All submissions must have a wet signature to be processed by the Board.
